Comunity Telemedicine

The purpose of the project is developing a community telemedicine organizational model that enables to improve control and prevention among citizen groups, in this case volunteers of the local day care centre of the Olympic Village.

The activities include the monitoring of the clinical variables through the use of sensors and devices that are not dedicated only to single individuals, thus significantly reducing all costs related to the purchase of equipment.

The system allows autonomous measurements, that are automatically transmitted to a remote server.

Sweetage 1

The project started in March 2009 and it enabled to create a prototype of a system for the remote monitoring of elderly chronic patients, who suffer mainly from COPD. Biometric measurements of specific vital parameters, which are calculated and transmitted to the Control Room located in the healthcare centre at the Bio-medical Campus of the University of Rome, enabled to promptly identify health deterioration symptoms, thus allowing doctors to preventively intervene in re-intensification phases and obtain evident benefits for the patients’ health.

The biometric and biomedical data collected can be integrated with other clinical, diagnostic and health information concerning the patients and then gathered together in one single open clinical record.

Centro Polifunzionale Territoriale in Massafra

The organizational module, which actively involves general practitioners of the local healthcare centre Centro Polifunzionale Territoriale in Massafra, intends to use the PHEBO® platform aiming at offering a constant, accurate and efficient assistance service through clinics and services.

The offer of services is enhanced by the provision of functions that use home tele-monitoring devices, thus ensuring prevention and management and accuracy of treatments to patients who suffer from chronic diseases: hypertension, diabetes, metabolic syndrome and COPD.

General practitioners access the platform and examine the data of each patient, then they create a personalized planning, scheduling visits and specifying the treatments that need to be followed, as well as setting the time and dose related to the drugs administration.

This way the General Practice undertakes the coordination of the prevention and assistance programmes, thus filling the gap between hospitals and local situations and ensuring a proper response to citizen health needs.

ProDomoSud

The project intends to develop an integrated technological model aimed at providing local assistance to vulnerable and disadvantaged people. The use of telemedicine is required to increase efficiency in the management of healthcare processes, by creating a technological infrastructure, which ensures the acquisition, the processing and the optimized sharing of the patient health information.

The test involves the ESS corporate network, the Digital Health Network, in collaboration with the charity organization Cittadella della Carità of Taranto, and it includes 3 solutions:

CHRONIC DISEASES TOTEMS based on the PHEBO technology, which will be installed in the Cittadella structures to periodically monitor the patients, who live or are hospitalized there.

Remote HOME MONITORING for clinic patients who suffer from multifactorial chronic pathologies.

POST-TRAUMATIC MOTOR REHABILITATION through an innovative sensory technology that identifies the motor parameters to provide an objective assessment of the patient recovery.

There is a double purpose: on the one hand, creating a Health Technologies Living Laboratory including house automation, telemedicine technologies, functional activities measurement and assessment systems, on the other hand, creating an integrated centre, which act as contact point for doctors, patients and technology suppliers, so that the development of health innovation processes is fostered.
